We are seeking a compassionate and dedicated Support Worker to join our team. The ideal candidate will play a vital role in providing high-quality care and support to individuals in various settings, including home care, nursing homes, and hospice care. Responsibilities

  • Provide personal care and assistance with daily living activities.
  • Support clients with mobility and transfers, ensuring their safety at all times.
  • Assist with medication management as per care plans and guidelines.
  • Engage clients in meaningful activities to promote social interaction and mental stimulation.
  • Implement behaviour management strategies to support clients with challenging behaviours.
  • Maintain accurate records of client progress and report any changes in condition to the nursing staff or care manager.
  • Drive clients to appointments or social activities as required, ensuring their comfort during transport.
  • Collaborate with healthcare professionals to develop and update individual care plans tailored to each client’s needs.

Requirements

  • Previous experience in care.
  • Certification in First Aid is preferred; additional training in nursing or related fields is an advantage.
  • Strong interpersonal skills with the ability to build rapport with clients and their families.
  • Excellent communication skills, both verbal and written.
  • Ability to work independently as well as part of a team.
  • A valid driving licence is essential for transporting clients when necessary.
  • A genuine passion for providing high-quality support and care to individuals in need.
